Go to https://welcome.fusiform.co and login using your Fusiform Vendor account. If you haven't had the On Boarding Tool configured for your account, email [email protected] to request set up.
The Dashboard for the On Boarding Tool is set up into two sections: Relationships and Invites. Relationships shows clinics that are ready to order your devices on Fusiform. Invites are clinics that you have invited to sign up for an account, but haven't logged in yet.
Click the New Invite button at the bottom of the page to begin the process of on boarding a new clinic.
Enter the email address of a clinician from the clinic you would like to on board. This clinician should be your primary point of contact at this facility and someone who is able to invite additional clinicians within the organization (How to Add Users).
Enter customer information that will help you keep track of this clinician relationship in Fusiform. The Customer Account Number can be an account number used within your organization to recognize this customer or a number that is created to specifically for Fusiform. Primary Contact should be the person you will be in touch with at the clinic during the on boarding process (likely the same contact as the email address).
Click Create.
A custom email is sent to the email address registered for this account prompting the primary contact to set up a password on Fusiform. Once a password is set up he/she will be able to automatically access your organizations devices to begin ordering on the platform.
Click the Return Home button at the bottom of the page to go back to the Dashboard.
The new clinician account information entered will be populated in Invites. Once the clinician logs in to Fusiform for the first time, this account will move from Invites to Relationships.
